The Program and Session Waiver Reports show the waiver and questionnaire responses collected during program registration, so you can confirm who has signed what for each program and session.
How to Read the Report
Each row ties a participant to a waiver question and the answer they gave, under the program and session they registered for. Family Admin Full Name shows the primary family member on the account when a family account is involved.
How to Filter
Filter by Program Name, Session Name, and Waiver Title to audit one program's waivers, or search a participant's name to confirm an individual signature.
Column Names and descriptions
Program Name | Name of the program. Programs: Private Lesson, League, Camp, Clinic, Class, Club Team. |
Session Name | Name of the session. Sessions sit under the program and can be broken down by skill level, season, age group, or other criteria. |
Waiver Title | Title of the waiver. |
Segment Name | Name of the segment. Segments (optional) add organization within a session, for example Mon/Wed vs. Tue/Thu groups or specific time blocks. |
Family Admin Full Name | Full name of the primary family member in the family account. |
Full Name | User full name. User: A global Bond account with a unique email that can be associated with multiple organizations. |
Question | The question asked in the questionnaire. |
